Make-Up Date for Senior High and Middle School Fall Sports Physicals
**Attention Students and Parents,**
We are pleased to announce the make-up date for fall sports physicals for Senior High and Middle School students. Please find the details below:
**Date:** Friday, August 9th
**Location:** Laurel Highlands High School
**Cost:** $10.00 (No cash accepted. Please make checks or money orders payable to Laurel Highlands Athletics)
**Important Information:**
1. Physical forms can be picked up at the security desks of both the high school and middle school.
2. All physical forms must be completed and submitted on August 9th.
3. All sections of the form must be signed where indicated.
4. Insurance information must be accurately entered on the designated lines.
5. Students without completed physical forms, necessary signatures, or insurance information will not be allowed to participate.
**Physicals Schedule:**
- **Senior High Candidates:** 8:00 A.M. to 9:15 A.M.
- **Middle School Candidates:** 9:20 A.M. to 10:50 A.M.
- **Late Arrivals:** 10:50 A.M. to 11:17 A.M.
All physicals will be conducted in the Nurse's Office at Laurel Highlands Senior High School.
